Text Box: ****************TIPS FOR CLIENTS WHO HAVE INSURANCE**********************

*Call your insurance carrier to verify the service is covered and ask whether precertification is required. (Insurance carriers call psychotherapy “outpatient mental health, office visit” or “professional” services.) Ask specifically about coverage, copays and deductibles for the provider you want to see, and ask if your copay is for a specialist if you see a psychologist.

*Ask whether your mental health benefits are assigned to a different company from your medical benefits. Many times a medical insurance company will hire a different insurance company to manage your mental health benefits. 

*If you are seeking family therapy (for yourself to be treated with other family members) be sure to ask whether your plan covers “family therapy” (excluded by some policies).

*Ask how many visits you are allowed per year and when your coverage year begins and ends

*Ask about deductibles and copays.  You may want to check how much of your deductible you have met so far specifically for in-network or out-of-network mental health care, depending on whether your provider is in-network or out-of-network.

*If the provider you want to see is an out-of- network provider, be sure to ask what your out of network benefits, deductibles, and copays are so you know in advance what you will be paying out of pocket should you choose to see an out of network provider

I am in network with and accept several insurance plans.

You can also privately pay on a fee-for-service basis if I am not in network with your insurance carrier.

 

I am in network with and accept the following insurances. I will file claims for these carriers.

             * Aetna      

             * Choice Colorado

             * Cofinity

             * CHP+

             * First Health/ Coventry plans    

             * Rocky Mountain Health Plans

             * Tricare (Prime, Standard, Select, Enhanced, Youth, CHCBP, and Tricare for Life)

             * Medicare (Note: I am a Medicare provider. However, if you have assigned your Medicare to UnitedHealthcare, Blue Cross Blue Shield, Cigna, Humana, or Kaiser/Value Options I may not be able to take your particular Medicare or you may incur out of network expenses—call the number on the back of your card to check whether your provider has to be in the network you assigned your Medicare to for care from that provider to be covered)

 

             I am a veteran and am happy to see VA clients if you obtain preauthorization under the Choice program and/or a fee-based agreement.

            

If you do not see your insurance carrier listed above, I am most likely not in network for your insurance plan. Please go to your insurance plan website or call your carrier and verify whether I will be in network or out of network for outpatient mental health services, and ask what your out of network deductibles and copays are.

 

*I am not in-network with stand-alone Medicaid. It is illegal for me to accept private payment for covered services if you have stand-alone Medicaid.

 

*I am not in network with Cigna, Magellan, ParkCare, Miners, Kaiser/Value Options, or Humana/LifeSynch. If you have out of network coverage with any of these insurance carriers, I am happy to file your claim once. You will need to call your insurer to find out what your out of network coverage and deductibles are, and you will need to file any claims they do not process the first time.

 

* I am not in network with Blue Cross Blue Shield, Multiplan, and United Behavioral Health/Optum (except that I AM in network for United Healthcare Military and Veterans/Tricare).  I am only able to see clients with BCBS or Optum/UBH who pay my fees in full up front. If you have out of network coverage, you may file your own claims with those carriers. If you do not have out of network coverage you may find that BCBS or Optum/UBH will not reimburse you at all.
